I have not really noticed any increase in spam since adding the DoFollow plugin. Most of the time when I do see an increase in spam it is due to some "anti-spam" article or if I head out and really start marketing the site. I use Akismet - I used to use a math validation plugin, but it kept breaking, so I gave up and simply use the "WordPress bundled" Akismet - It seems to work fine for the most part.
